2. How would you like the "Payable to:" line to appear on your Gift Certificates?
   Blank "Payable to Bearer" Personalized, with an individual name on each certificate (15¢ extra per certificate)
   If you choose Personalized, please include a single-column list of the names (no titles, departments, etc.) in both
   an electronic format (MS Word or Excel preferred) and a hard copy. No name can be longer than 25 characters.
3. Determine your service charge.
   The service charge per certificate is $0.75 for non-personalized certificates and $0.90 for personalized certificates.
   Note, for orders of less than 20 certificates the service charge is $18.00
